1. Call our office at (607) 277-2570 or fill out our contact form to schedule a convenient appointment time. Our usual office hours are

• Mondays and Wednesdays, from 8 AM to 6 PM
• Tuesdays from 1:00 PM to 6:00 PM and Thursdays from 10AM to 1PM.
• Fridays from 8 AM to 5 PM.

3. Print out the links below, and bring them with you. We use a wonderful, comprehensive health history form to learn all about you and your health concerns. To save us both time, we want you to bring it in after having already filled it out in the comfort of your home.
